Renewal request application

Please note we have simplified the renewal application process.  Complete the materials below, save them in PDF, Word or Excel format and email then as attachments to reports@nyf.org.   

A complete renewal application must include the following:

• Cover sheet

• Narrative

• Financial documents and other supporting materials

Once your information is received, there is very little time for us to correct it.  Please make sure that the materials are complete and accurate.

Don’t hesitate to call your program officer if you have any questions as you prepare your financial materials.  Incorrect or inadequate preparation of the fiscal materials can result in your grant request not being considered.
